Here’s the Short Leg Checklist you need to help identify the best course of action when there’s a chance one leg is shorter than the other.

Is it Functional?

Perhaps it’s Structural.

Functional? Structural? Which is it? IT MATTERS!!!

Functional Short Leg CHECKLIST

NOTE: Functional short legs “get better” (balance in length) quickly AND stay better!

TOP SIGNS of a Functional Short Leg

  1. Recurrent Observed Leg Length Deficiency (LLD)
    1. Lying Down (face up &/or face down assessment)
      1. Medial malleolus 
        1. that’s the inside heel bone 
        2. does one bony landmark appear contracted/higher-up compared to the other?
      2. Tape measurement
        1. umbilicus (belly button) to medial malleolus
        2. anterior superior iliac spine to medial malleolus
          1. that’s a bony landmark on each side of your pelvis
    2. Standing Assessment
      1. Superior-most ilium
        1. hands on the top of the pelvic bones
          1. not very accurate; “which one looks higher?”
      2. Tape measurements
        1. umbilicus to medial malleolus
        2. ASIS to medial malleolus
  2. Persistent/Chronic Observed Compensatory Postures
    1. Anterior pelvic rotation (+/-Ry)
      1. 70% of cases to the short leg side
    2. Lateral thorax translation to the short leg side (+/-Tx)
    3. Laterally flexed thorax away from the short leg side (+/-Rz)
    4. Lateral head flexion back towards mid-line (+/-Rz)
    5. Unilateral ankle inversion/eversion (+/-Rz)
    6. Unilateral foot internal/external rotation (+/-Ry)
  3. Signs of Asymmetrical wear & tear Osteoarthritis  
    1. Degenerative Joint Disease (DJD) revealed via history & consult
    2. Osteoarthritis/DJD visualized via specific imaging of the foot, ankle, knee or hip
    3. Big toe arthritis, bunion, hammertoe, plantar fasciitis, heel spur

TOP SIGN – Structural Short Leg – ACTUAL EVIDENCE

LLD examine the evidence

  1. Plain Film Radiographs – Gold Standard Assessment
    1. X-ray is the gold standard for both valid and reliable evaluation of LLD when standardized protocols are followed
      1. Modified Fergusons x-ray with the following exception:
        1. Steep sacral base angle (SBA)
          1. Measured on the lateral x-ray
          2. Visualized shallow pelvic bowl AP view
      2. Ferguson view
        1. Tube tilt-up to match measured SBA

Every problem associated with a functional short leg is also present with a structural.

In other words, the osteoarthritis and degeneration associated with a functional short leg is still happening.

So are the postural compensations and observed leg length changes. 

The MAIN DIFFERENCE is…

Structural short legs Never, EVER, get better when treated as functional!

End of story.

Folks, there’s absolutely NO reason to suffer with endless and medically unnecessary treatments for a functional short leg when your problem is actually structural!
